Vehicle History & Modification Worksheet
Write down what you know before a diagnostic appointment. Unknown is an acceptable answer. This is not a diagnosis, and the worksheet is not submitted or stored by the website.
Why this matters: Previous accidents, bodywork, accessories, repairs and age can guide a test plan, but none proves a cause without evidence.
01 / Vehicle year, make, model; approximate mileage
02 / How long owned? Bought new or used?
03 / What is the concern? When first noticed?
04 / Accidents, panel replacements, body/paint repairs?
05 / Added accessories or aftermarket electrical work?
06 / Previous repairs, parts, wiring changes or programming?
07 / Exposure to salt, flooding, moisture or corrosion?
08 / When does it happen: wet, cold, bumps, brakes, towing?
09 / What has been tested or ruled out?
10 / What records or dates could help? (No personal identifiers needed)
ATD principle: History helps select tests. Measurements, suitable inspections and reliable documentation establish what is actually known.
No form data is collected. If you contact ATD, avoid sending VINs or sensitive customer records until an appropriate sharing method is agreed.
